Once printed, please complete the form, and fax it back to us on 0871 434 1837. Once we have received these forms from you, a member of our accounts team will contact you to confirm your account is open. Thank you for taking the time to complete the Teleticket account application forms & we look forward to working with you... Please note that all 3 forms must be received before an account can be opened. Direct debit is a mandatory requirement. |
